SUBJECT: Request for Transfer

DATE: 09 DECEMBER 2022

1. The undersigned respectfully request for transfer of assignment from

Talaingod Fire Station, Davao del Norte to Hagonoy Fire Station, Davao del Sur, Region
XI due to the following reasons, to wit:
a) That the undersigned is a resident of Purok Adelfa, Barangay Tres De
Mayo, Digos City, Davao del Sur and traveling more than 131 kilometers
and approximately 3-4 hours in between four (4) cities and five (5)
municipalities;

b) That the undersigned is experiencing financial constraints due to travel

expenses;

c) That the request to transfer is not motivated by ill-feelings towards

anybody and solely on his own sincere and honest desire to seek
reassignment and place where he is more familiar and more convenient
to be.

2. The undersigned fervently hopes and prays that this request for transfer be
granted.
