CREW MEMBER – NAME/TITLE
DATE/TIME INITIAL REPORT

Crew member absent without leave (e.g. not reporting for duty or returning by stipulated curfew) –
Ensure Yacht has:
1 Call and text mobile number (if applicable)
2 Establish time line and last movements of missing crew - interview other crew & consult
logs
3 Review onboard CCTV
4 Advise local agent on situation – enquiries made to local authorities & hospital
5 Consult marina/port – check with facility security, review CCTV footage
6 Search crew members cabin* and review social media for further information
7 If practicable visit area of crew members last known movements
*If practicable, photograph cabin & any items that may be relevant or removed

AFTER 3 & 6 & 12 HOURS – Ensure Yacht has:


1 Advise/Update DPA & GM
2 Review time line and last movements of the crew member
3 If practicable, trace route missing crewmember took whilst ashore with ‘search party’
4 Contact local agent for update from local police and/or hospitals
5 Contact Port/Marina for update
6 Review social media for any updates & attempt to contact crew member by telephone
7 Maintain a log of events and actions taken (a full report may need to be compiled
pending outcome)

AFTER 24 HOURS – Ensure Yacht has:


1 Advised/Updated DPA & Yacht Operations Manager
2 Contacted local agent
3 Made formal missing persons report to local authorities in line with local protocols
4 Taken formal statements from crew
5 DPA or Master to notify Flag State
6 Make entry into Official Log Book
7 Master or Office to contact NoK – when was their last contact?
8 Master or GM to make contact with Insurance Broker (possible P&I claim)

MORE THAN 48 HOURS/IF LEAVING PORT BEFORE MISSING PERSON IS FOUND – JLS Yachts to:
1 Make sure person/s appointed will keep case moving forward. This can be agent, P&I
correspondent or another person.
2 Make sure police and others involved have full set of contact details for JLS Yachts and
yacht.
3 Consider flying next of kin to port of disappearance. May also consider leaving a
competent crew member behind if helpful.
4 Consider if there is a media issue.
5 If leaving port leave a bag of personal effects and passport with appointed person

IF MISSING PERSON IS FOUND DECEASED – JLS Yachts TO INFORM:

3 P& I Insurance
4 Flag State
5 Police (if not already aware)
6 Class (if accident)
