SUBJECT : Request for Additi onal Safety Equipment

Dear Sir,
We would like to request an additional Personal Protective Equipment (PPEs) for our newly
hired employee to be used during routine site inspection especially while the construction is
still going on. Also, the PPEs will be useful during punch listing activities for equipment and
facilities for turn over as well as during hands on training in operating such equipment
testing.

To manage safety at all times, the PPEs will be a mandatory requirement on site. Listed
below are the items for your approval:

1. 9 pairs safety shoes

2. 7 pcs Reflectorized Vest (White)
3. 7 pcs Hard Hat (White)
4. 7 Pairs Ear Plugs
5. 7 pcs Safety Eye Glass
6. 8 Pairs Rubber insulating gloves class 00 for high voltage
7. 8 Pairs Leather Gloves
8. 5 Long Shackle Padlock for pad mounted transformer
