APPLICATION PROJECT 2: THE PHARMACY AND THERAPEUTICS COMMITTEE (P&TC)

I. OBJECTIVES
A. Understand the importance of Pharmacy and Therapeutics Committee
B. Evaluate the policies, guidelines, and procedures in choosing members of the
Hospital’s P&TC
C. Assessment of the policies, functions and scope of the Hospital’s P&TC
D. Evaluate the objectives, policies, guidelines and procedures of the Hospital’s
formulary system (P&TC)
E. Apply competencies in developing formulary system (PTC)
F. EvaluatetheroleofPharmacistinpharmacovigilanceandrationaluseofmedicines

II. ACTIVITIES:
A. Illustrate the organizational chart of the P&TC

B. Make a Schematic diagram on how to report ADRs.

C. Create and submit a simple formulary system based on the criteria learned in
the selection of formulary drugs. List only five drugs in each drug therapeutic
category. Use the Philippine National Formulary as your reference guide. Attach
or paste in the space provided or submit it as google document.
