I, the undersigned, request your good office to kindly issue Police Clearance Certificate
to RESHMA S R , Date of birth: 06-03-1992 bearing Aaadhaar Card No.
for the purpose of obtaining Nursing Registration for to be able to
practise as Nurse in the State of Maharashtra, as per provisions in The Maharashtra Nurses Act,
1966 and above reference.

Kindly issue the same at the earliest so as to prevent delay in further process of Registration.
