AFFIDAVIT OF COHABITATION

We, SUNDAY MARTINEZ TADEM and RECHELIE LIMBAGA RAMOS,

Filipinos, both of legal age and residents of Brgy. Buenavista, Tandag City, Surigao Del
Sur, state under oath, THAT:

1. We are living as husband and wife without the benefit of marriage for already
six years now;

2. During our cohabitation, we ourselves have no legal impediments to marry

each other;

3. On that common-law relationship, we are gifted with one (1) child, a female, in
the name of DEVIEJANE RAMOS TADEM who is 3 years old now;

4. We are executing this affidavit to attest the truthfulness of the foregoing facts
and for whatever legal purpose this may serve best.
