This turnover of Duties and Responsibility made and declared by and between:
LUMGAR SECURITY SEVICES INC., with office address at Room 203 S & A Bldg. 3
Cambridge St, Cor Aurora Blvd Cubao, Quezon City
And

_________________________________________________________, herein referred to as
the “Incoming Security Agency”
Witnesseth

WHEREAS, the Outgoing Security Agency after having been formally notified to the termination of
security services at the essential post of PARK SOLUTIONS INC. (PSI) located at CABANATUAN
CITY CENTRAL TRANSPORT TERMINAL effective 01 February, 2023.

WHEREAS, in the presence of the posting procedures, the duties and responsibilities for the
protection of property is hereby turned-over to the incoming security agency without sign of
discrepancy with ALL SECURED STATUS.

NOW, therefore, after having been accomplished this form for turn-over of duties and
responsibilities, the Incoming Security Agency shall now assume the duties and responsibilities from
Outgoing Security Agency.

WE, the duly authorized representatives of both parties hereby affixed our signature below to attest
for the truthfulness of the above facts.
