    Global PT Water sample Z – score in Sulphate of water

    Root cause Analysis. : As Per the instruction, by PT Provider 20 ml given Sample to be dilute
    upto 500ml i.e. if 20ml sample has taken, then 480ml of D.M./ Distilled water to be added to the
    sample. But during dilution 10ml sample has been taken & it was diluted with 250ml water instead
    of 240 ml water, which may lead the higher value of saulphate content.

    Corrective action: As Dilution was done previously by using 250ml D.M. D.M./ Distilled water
    with 10ml Sample, further for retesting purpose once again remaining 10ml sample has been
    dilution with 240ml of D.M./ Distilled water & further retest the sample.

    Preventive action: The dilution process the given sample shall be followed, as per the instruction
    & for that, the dilution proportion will be mentioned in raw data sheet (Laboratory observation
    Sheet).
