I, EDRINE E. PETALCORIN, Filipino, of legal age, SINGLE and a resident of Bauk-Bauk,

Balingoan Misamis Oriental, after having been sworn to an oath in accordance with law, do herby depose
and say:

That I am applying for Administrative Assistant III at Division of Misamis Oriental;

That as such, I was required to submit in support of my application for employment with the
Department of Education;

That in compliance with the requirements, I have submitted copies of the documents, to wit:
