AFFIDAVIT OF PERSONAL SERVICE

I, MICHELLE ANNE P. GAYLON, of legal age, Filipino, after having duly

sworn to in accordance with law hereby depose and state that:

1. On the 2nd day of December 2022, for and in behalf of Ms

Evangeline Barolo, I went to Block 2, Lot 15, Bagwis Village, Las
Pinas City, to personally serve the Final Demand Letter Prior to
Legal Action to Mr. Eric C. Amador who is occupying the house in
the said address.

2. That Mr. Amador was given a copy of the Final Demand Letter but
refused to sign the receiving copy.

3. That I have executed the foregoing to attest to the truth hereof and
for whatever legal purposes or intents this may deem worth.
