SWORN STATEMENT

I, ATTY. ABCDE, of legal age, Filipino, with office address at

Purok ____, __________, Misamis Occidental, Philippines, after having
been duly sworn to in accordance with law hereby freely depose and
say that:
1. I am the handling counsel of WXYZ, the Petitioner in Civil Case
No. _____________ (For: Declaration of Nullity of Marriage in
accordance with Paragraph (4) of Article 35, in relation to Article
40 and 41 of the Family Code), entitled “ Petitioner v.
Respondent”;

2. I have personally verified Petitioner’s residency, specifically at

that the Petitioner had been residing thereat, for more than six
(6) months prior to the filing of the instant petition;
