1. SCOPE
This task risk analysis describes the preventive measures that shall be taken when NDT activities are executed in risky
circumstances

2. REFERENCE DOCUMENTS
HSE RISK ASSESSMENT AND EVALUATION OF SGS BELGIUM NV : SARIER METHOD

3. RESPONSIBILITIES
It is the responsibility of the QHSE to keep up to date this document. It is the responsibility of the NDT practitioners to
apply the preventive measures during the intervention.

4. EXPLANATION SARIER METHOD

"SARIER stands for Systematic Analytical Risk Assessment, Evaluation and occupational risks Registry.
In each risk assessment we will try to make an inventory of the dangers that can cause health damage and we will try
to estimate a weighting of the risks. The major advantages of the SARIER method are that:

 It uses a risk graph

 Used values are better defined as Kinney method
 It is limited to 5 levels
 It takes into account number of people exposed and conditions
 Measures are divided, depending on their risk mitigation influence, this means that we estimate for the risk
graph :
o Number of exposed x exposure duration -> Exposure
o Exposure x conditions -> Probability
o Severity x probability -> Risk level
The SARIER systematically takes into account three different reduction levels:
 Organizational measures reduction: 0.80 Examples: procedures, toolboxes, training, PPE, icons,
 Technical measures reduction: 0.60 Examples: detection equipment, fire extinguishers, emergency stop,
 Infrastructural measures: reduction: 0.40 Examples: shielding, containment, isolation,…

6. ORGANISZATIONAL MEASURES AND SUPPLEMENTARY MEASURES FOR RADIOGRAPHERS

Organizational measures (OMTA) 1) Everyone should have the latest version of the HSE work instructions with them (or in the car)
applicable to each work station 2) Everyone shall have knowledge about the latest HSE instruction applicable.
3) Everyone shall always proactively perform an LMRA (this means not only at the start of works, but also after a break or change of working conditions) and
to be alert during the works.
4) Everyone needs to wear/to have the for him applicable min. required (+ specific) basic equipment.
5) Everyone needs to be physically / medically fit.
6) It is absolutely prohibited to start work or drive under the influence of drugs / alcohol (cf. Sgs 12 safety rules)
7) Min. to have valid Basic SCC (or should get it into 3 months employment)
8) Order and cleanliness at the workplace is still applicable.
9) Any SGS employee shall know the 12 sgs lifesaving rules and apply them correctly
10) Safety observations would be regularly performed by BUM / Operation Manager / QHSE / project manager
11) Each technician receives annually min. 10 toolboxes, spread over 12 months
12) Every year a global SGS safety day is organized.

Supplementary measures (MTR) 1) Radiographic testing will only be performed by trained and certified personnel. Radiation safety is an integral part of training or certification.
applicable to all radiographers 2) All of the occupationally exposed personnel should have before employment, and in accordance with legislation at least 1 time per year an
instruction about the rules regarding radiation safety.
3) On site radiographic testing will always be performed by two technicians working for SGS. One of the two persons will monitor the radiation/barrier zone.
One man principle can only be applied if the radiographs are taken in a physical control approved radiation bunker used with the operating conditions of
the radiation bunker.
4) The technician receives a medical check conform WI 28 002
5) The radiation zone is marked according to the conditions of the permit and as described in the work instructions. (Visible with yellow / black ribbon with
radiation symbols)
6) Radiation intensity X-ray is 25 to 100 times greater than that of an isotope
7) At a test shot or at the 1st recording the radiation intensity at the perimeters around the radiation zone will be actively and consciously measured by
the radiographer or helper.
8) The second man (assistant) will basically guard the perimeter around the radiation zone to prevent unauthorized people to come in the radiation zone.
9) After work, the dose incurred will be reported through the work assignment to SGS.
10) Planning knows when and where the radiation sources are present. This is necessary to inform all parties involved.
11) The RX equipment are warmed up with a protection or with the radiation beam directed to a safe place (i.e. to ground).
12) The equipment is regularly maintained in accordance with current calibration and maintenance procedures
13) The chief radiographer (or assistant) will take at urgent measures the necessary steps as described in the emergency procedure.
14) Proper lifting technique to manipulate radiation source / use available charts

- Roofs with a small residual wall thicknesses (<3 mm) will not be entered.
- It is not allowed to enter roofs during storm, snow or sleet weather
- Additional permit requirements should always be followed
CONDUCTIVE 15.7 - Extra electrocution hazard - The inspection lamps are equipped with a 2-pole safe CEE plug with purple color
SMALL CONFINED by using portable lamps. ring (<24 V)
SPACES / The required SELV is achieved through a safety transformer which in any case must
be placed outside the enclosed conductive area. In tanks, etc. where the concept
''SMALL” is not applicable and where large cable lengths are needed the safety
transformer may be installed inside the tank (Only possible if the cables are
performed of a double insulated version (Art. 94))
- Extra danger for - Other portable equipment
electrocution of portable - Devices to SELV (Safety Extra Low Voltage) 24 V
equipment - Appliances with double insulation (class II), with the mark to 230 V. These devices
may not be grounded. They should lined up via a protection transformer 230/230 V
outside the confined space.
- Devices of Class I at 230 V
Only if these devices in double-insulated version does not exist. These devices in Class
should individually fed via a protection transformer 230 / 230V. This transfo should be
placed outside the narrow conductive area.

hook.
- Temporary work floors in tanks will be accessed only if the floors are closed and
railings are provided.
- Harness are available and are periodically checked by an authorized body.
- When wind load > 6 Baufort all work at height should be stopped.
Fall of objects. - Use of carrier bags
- Only visible checked and not expired hoisting ropes may be used
MANUAL WORK 15.13 Injury to hands and fingers - Wearing appropriate gloves when handling test pieces (regardless of size),

ceramic fibers 1. The potential health hazards, including some additional risks due to food
and smoking;
2. precautions to prevent exposure;
3. hygiene requirements;
4. wearing and use of protective clothing and personal protective equipment
- Facilities:
Defining a zone: working with ceramic fibers
On the edge of the zone of "working with ceramic fibers" is a changing room
provided, equipped with:
Sink for cleaning PPE / garbage cans/ adequate ventilation the oven shall be in
underpressure(chimney effect) Local air extraction
work at low load to ceramic -Minimum following PPE:
fibers 1. Standard PPE such as helmets, safety glasses, safety shoes / boots, and gloves;
2. Disposable tyvek (type 5, 6: protection against dust and particles). The cap
must be worn on the head and under the helmet;
3. Use a disposable FF P3 filter
4. The safety harness must be worn UNDER the disposable overall.
- When leaving the zone "working with ceramic fiber" pull out the contaminated
clothing and place it in trash cans equipped with RCF label. Subsequently, everything
that has been in contact with ceramic fiber such as helmet, goggles, shoes need to be
cleaned. Wash hands and face .
BAD WEATHER 15.16 Work during heavy rain - See WI 28.005. In case of doubt: call supervisor and/or contact the customer
