11 GORDON’s FUNCTIONAL HEALTH PATTERN

HEALTH PERCEPTION AND HEALTH MANAGEMENT 

Before:  
“Wala akong pakialam sa health ko kaya inom lang ako ng inom at
naninigarilyo din ako” as verbalized by the patient. The patient has no known
allergies. The patient used herbal medicine such as oregano, paragis, tawa -
tawa and malunggay. The patient buy and take over-the-counter drugs such as
analgesic. 
During: 
Because of his condition, the patient thinks he is no longer healthy. The
patient regrets about his previous lifestyle and he became aware of his
condition. The patient no longer uses herbal medicine. 

NUTRITIONAL METABOLIC  
Before: 
The patient stated “Nung maliit ako hindi ko nakakain ang gusto ko kaya’t nug
kaya ko nang bilhin ang gusto ko hindi ko nilimitahan ang sarili ko sa
pagkain.” He usually consumes, meat, instant noodles, acidic beverages (e.g.,
coke, cobra) and alcohol, and drinks 5-6 glasses of water. He does not have
any allergies. 
During: 
He eats 3x a day, he eats vegetables and foods that have no salt, and not
sweets. His intake of water is 350ml in 12 hours and in the evening is half a
glass of water. 

ELIMINATION 
Before: 
The patient usually voids 5-6 times a day, the color of the urine is light yellow.
He defecates every morning, the color is brown. He does not have any
problems in voiding and defecating. He never used any chemical laxatives and
stool softeners. 
During: 
Patient is place in Foley catheter. The patient urine is estimated ranging from
100 to 150ml. The color is yellow. He experiences problem in defecating, the
stool is color black with blood. 

ACTIVITY/ EXERCISE 
Before:
He stated that before his hospitalization his physical exercise was walking,
basketball and riding a tricycle. 
During:
The patient stated that he becomes weak in prolonged activities, he can only
lay on his bed because when he walks, he gets tired immediately because of
his condition. 
COGNITIVE PERCEPTUAL 
Before 
The patient is oriented to people, time, and place responses to stimuli verbally
and physically. He can speak and understand Ilocano and tagalog. His
educational attainment is high school graduate, and he can read and write. 

During 
The patient is oriented to time and place was able to respond appropriately and
correctly to questions, retaliates as possible and no sensory deficit, he is aware
on his condition. 

SLEEP/ REST 
Before 
The patient usually sleeps 8-9 hours, his earliest time in going to sleep is at
8:00 pm and he wakes up at 5:00 am. Sometimes, he takes a nap at noon for
about 1 hour. He doesn’t have any difficulties going to sleep and doesn’t use
any medication to promote sleep. 
During 
Due to his health condition and difficulty falling asleep, he is unable to sleep
as much as he used to. 

SELF PERCEPTION/ SELF CONCEPT 

Before 
The patient can express his feelings about his condition, he feels annoyed
about his condition, but he wants to feel that he is still strong; he can do
whatever he wants. 
During 
Patient feels weak, he learned his lesson and increased willingness to take care
of himself. 

ROLE/ RELATIONSHIP 
Before 
The patient plays the role of a father to his child and a husband to his wife, he
is the provider on their family, the patient feels the support of his family
especially his partner.  
During 
The patient has a good relationship with his family, the patient is well
supported by his family. He receives a positive reinforcement and provides
him comfort and reassurance. Now they are relying solely on their small store,
because he cannot provide for his family. 

SEXUALLY REPRODUCTIVE  
Before 
The patient is sexually active, he has a child in college, high school and
elementary. He has no history of sexually transmitted disease or any disease
affecting his genitals. 
During 
 The patient and his wife claimed that they are sexually inactive due to his
condition. 
 
COPING/ STRESS TOLERANCE 
Before 
The patient copes up with stress by he seeks support from his friends and play
a basketball, He doesn’t inform his wife, but when it comes to his health he
does. 
 
During 
The patient verbalized that now that he is stressed because of the hospital
expenses, the only thing he does to get rid of it is to use his cellphone. 

VALUES BELIEF 
Before 
Our patient’s religious affiliation is Jesus is Lord, he goes to church
occasionally he brings his wife and his children. 
 
During 
He is not active because of his condition, but always prays to God for wisdom
and safety and he never blames God for his condition; the patient’s
relationship with God remained unchanged. No restrictions in the procedure
brought by religion.
