1. Introduction and Purpose of the Study

The researchers aim to find out the Effectiveness of Blended Learning
Modality to the Adaptability of Students’ Understanding of The Lesson in BRHS SY
2021-2022 among the participants.

2. Description of the Research

During the survey, you will be asked to go to a certain place which is way
comfortable and quiet place to stay in to make the survey session clearer. You will
also be asked to be mindful on answering all the time.
Meanwhile, the interviewer will be alone throughout the survey process, to
ensure the confidentiality of the information that you will share. The interviewer will
use a survey questionnaire with the sets of statements.

3. Subject Participation
The researcher estimates that 30 to 50 Senior High students will participate in
this study.
The qualifications for the participants in this study are:
a. a Senior High student of BRHS enrolled during S.Y. 2021-2022; and
b. experienced blended learning modality during the above-mentioned school
year.

4. Potential Risks and Discomforts

 No known risks.

5. Potential Benefits
The data that will be collected and eventually analysed will be useful for
everyone in understanding and finding out the Effectiveness of Blended Learning
Modality to the Adaptability of Students’ Understanding of The Lesson in BRHS S.Y.
2021-2022 among the participants.
The results might also be used by the private and public sectors in formulating
an action plan for the mentioned matter during such times of pandemic.

6. Confidentiality
All information that will be gathered during the interview will be kept
confidential. I will not be sharing any information to anyone outside the research
team. Your name will not be disclosed on the papers containing the information you’ll
 be giving us, instead, an alias or a code will be used, and only the researchers will
know about it to ensure your safety.

7. Compensation
The participants will not be compensated nor receive any monetary payment
for their participation in this research. However, words of appreciation for helping this
study will be guaranteed by the end.

8. Voluntary Participation and Authorization

Your participation in this research is VOLUNTARY. It is up to you to decide
whether or not to take part in this study.
If you decide to participate, you will be asked to sign the consent form. If you
decided not to participate, it will have no bearing on anything other than this study.
Your decision will have no impact on your study.
And even if you initially decided to join in the study then eventually changed
your mind, you are free to withdraw your participation and the researcher will respect
your decision.

9. Withdrawal from the Study or Withdrawal or Withdrawal of Authorization

You have the right to withdraw any time even if you already consented to
participate. You are not required to engage in this study if you do not want to, and
your decision to do so won’t have an impact to your career and or in finishing your
study.
By the end of the survey session, if you do not agree with the interviewer’s
notes or if you think you were misunderstood, you will be given the opportunity to
review your remarks and change or omit parts of them.

10. Cost/Reimbursements
There is no cost for participating in this study. Any kind of expenses resulting
from participation in this study will not be reimbursed by the researcher.
