    Dear Sir/Ma’am,
    Good day!

    We, the Senior High School Students under General Academic Strand 12 would like to ask for
    permission to allow us to conduct a survey among the Senior High students in pursuit of the
    success of the objective of our study, entitled, “Effectiveness of Blended Learning Modality of
    Students’ Understanding of the Lesson in BRHS S.Y. 2021-2022”.
    The survey will only last about 15 to 20 minutes and will only be conducted under the school
    premises and dependent to the availability of the participants. Participation in the survey is
    entirely voluntarily and there is no coercion intended. Rest assured that the data gathered will
    remain confidential and for academic purposes only.
    We are hoping that this request will merit your favorable approval. We appreciate your interest
    and help with this research in advance.
