PARENTAL CONSENT

_____________________________________________, would like to send my son/daughter to
the upcoming Regional Geodetic Engineering Students' Summit 2023, which will be held
on March 27-28, 2023, at the Bicol University East Campus in Legazpi City, Albay with the
theme “Follow Your Inner Compass”, having a Registration fee of P1,500.00, accompanied by
the GEP-NCF SC Officers and Engr. Joshua Bulalacao. I understand and agree that the organizers
will do their best for the safe and smooth conduct of the 2-day event. However, please note that
due to the event possibly ending late, on March 29th we will return to Naga City. Still, in case of
any unnatural happening, I will not hold the Naga College Foundation, Inc. College of Engineering,
and the officers of Geodetic Engineers of the Philippines, Inc. - NCF Student Chapter responsible.
