solemn affirmation as under:-

We hereby state that we have married on 11th June 2017 at

Shankar Mandir, Vikhroli Parksite, Vikhroli West,Mumbai-400079 as
per Hindu Marriage Act, 1955 or Special Marriage Act 1954 or
Marriages are performed under any law in force.

We hereby further state that, we are neither registered our

marriage nor have applied for registration of marriage before the
Register of Marriage or any authority in force.

We are swearing this Affidavit to produce before Register of

Marriage to record the above mention fact as stated herein above.

We hereby further declare that the information provided above is

true and correct to the best of my personal Knowledge, information
and belief. We fully understand the consequences of giving false
information. If the information is found to be false, we shall be
liable for prosecution and punishment under section 199,200 of
Indian Penal Code Section and/ or any other law applicable thereto
