AFFIDAVIT OF LOSS
I, SHERYL ANNE B. UMALI, of legal age, single, Filipino, and a resident of Brgy. 5 San Ramon
Vintar, Ilocos Norte, after having been sworn to in accordance with law hereby depose and say
that:

1. I am a holder of a Postal ID issued by the Philippine Postal Corporation (PhilPost) in

Rodriguez, Rizal.

2. On 19 November 2020, at 11 am, my wallet was lost/stolen during the Sirib Awards held
at Ilocos Norte Development Office (INYDO), Centennial Arena, Laoag City.

3. All my valuable items in my wallet, including the Postal ID issued to me by PhilPost,

were lost/taken.

4. I was not able to find my valuable items and my Postal ID despite diligent efforts to
locate or retrieve them.

5. I execute this affidavit to attest to the truthfulness of the foregoing information, and to
secure another postal ID at PhilPost, and for whatever legal purpose it may serve.
