Each item found in violation is reported below along with the code requirement.
Inspector Findings: Observed large quantities of mouse droppings throughout the facility. See violation 14A for more
details. One 50 pound bag of flour in the bagel prep area that was adulterated due to mice chewing
through the food item. Correction: Operator discarded the bag of flour at the time of inspection.
**Note: Operator states that Quality Pest Control is on contract for extermination services.
Exterminator must be contacted immediately to address the infestation. Facility was closed at the
time of inspection. Facility can reopen once mouse issues have been remediated.
Code Requirements: Toxic chemicals are improperly labeled, stored or used so that contamination of food can occur.
Inspector Findings: Cleaning chemicals and hand soap stored above food items on the shelves next to the hand wash
sink in the bagel prep area. Correction: Operator moved the chemicals to a

Inspector Findings: 1. Wire racks in the produce walk-in cooler in the front kitchen prep area are rusted.
2. Door gasket on the walk-in cooler in the bagel prep area is in disrepair.

Inspector Findings: Cutting board next to the bagel rolling machine is heavily stained and scored.

Inspector Findings: 1. Several light shields are soiled with debris throughout the facility.
2. Air conditioner above the three bay sink is soiled with dust.

separated for food service operations, live animals, birds and pets not excluded
Inspector Findings: Bagel prep area is cluttered with tools, clothing, shoes, empty boxes, and various unused items.
Placard of closure applied to the doors of the facility on this date. **Note: Do not remove the placards until the
facility is cleared for reopening by MCPHD.
Reinspection is to be completed once cleaning and sanitizing of all affected areas is completed and
extermination invoices are received.
