ITEM # 1H WAS FOUND IN VIOLATION 1 TIME(S). CRITICAL VIOLATION
Part or parts of the item were observed to be in violation which were corrected at the time of inspection.

Code Requirements: Food from unapproved source, spoiled, adulterated on premises.

Inspector Findings: Observed large quantities of mouse droppings throughout the facility. See violation 14A for more
details. One 50 pound bag of flour in the bagel prep area that was adulterated due to mice chewing
through the food item. Correction: Operator discarded the bag of flour at the time of inspection.
**Note: Operator states that Quality Pest Control is on contract for extermination services.
Exterminator must be contacted immediately to address the infestation. Facility was closed at the
time of inspection. Facility can reopen once mouse issues have been remediated.

Code Requirements: Toxic chemicals are improperly labeled, stored or used so that contamination of food can occur.

Inspector Findings: Cleaning chemicals and hand soap stored above food items on the shelves next to the hand wash
sink in the bagel prep area. Correction: Operator moved the chemicals to a

different location away from food items. **Note: Education provided on proper storage of chemicals
and cleaning supplies.

Inspector Findings: 1. Wire racks in the produce walk-in cooler in the front kitchen prep area are rusted.

2. Door gasket on the walk-in cooler in the bagel prep area is in disrepair.

3. Door gasket on the prep cooler at the service line is torn.

4. Cardboard lining shelves behind the service counter.

Inspector Findings: Observed large quantities of mouse droppings throughout the entire facility in all prep kitchens, the
service area, basement storage areas, on storage shelving, on a bagel rolling machine, on a bagel
toaster, on a bagel slicer, and in a plastic bin containing a

sealed bag of malt powder. Observed two deceased mice on a glue board under a desk behind the
front service counter and one deceased mouse on a snap trap behind the hand wash sink in the
kitchen. Facility is on contract with Quality Pest Control for services. The most recent invoice for
extermination services of the facility is from 1/19/23 via Exodus Exterminating Inc. **Note: Clean
and sanitize all affected areas. Operator is to contact an exterminator ASAP to address the mouse
infestation. Look to the comment section for further information.

INADEQUATE INSECT/RODENT CONTROL

Inspector Findings: 1. Missing/broken base boards throughout the facility.

2. Heavy accumulation of corn meal on the floors in both bagel prep area walk-in coolers.

3. Wall in bagel prep area behind shelving is soiled with food debris.

4. Hole at the bottom of the wall/missing cove molding near the front window.

5. Ceiling next to the bagel oven is actively leaking with electrical wires nearby.

6. Ceiling tiles throughout the facility have holes in them.

Inspector Findings: 1. Several light shields are soiled with debris throughout the facility.

2. Air conditioner above the three bay sink is soiled with dust.

Code Requirements: Premises littered, unnecessary equipment and article present, living quarters no completely
separated for food service operations, live animals, birds and pets not excluded

Inspector Findings: Bagel prep area is cluttered with tools, clothing, shoes, empty boxes, and various unused items.

Additional Information Collected During Inspection


Comments: Food service establishment has been closed at this time by MCDPH due to mouse droppings on food making
equipment and food prep surfaces, as well as adulterated food. FSE will need to hire a licensed exterminator
to eliminate mouse infestation. Forward all invoices of services, including snap counts, to
food@monroecounty.gov. See violations 1H and 14A for full details.

Placard of closure applied to the doors of the facility on this date. **Note: Do not remove the placards until the
facility is cleared for reopening by MCPHD.

Reinspection is to be completed once cleaning and sanitizing of all affected areas is completed and
extermination invoices are received.
