PLEASE ATTACH YOUR VENDOR FEE TO THIS FORM TO EXPIDITE PROCESSING. Space is limited with only 130 spots available and acceptance is subject to the approval of the Arizona Parks and Recreation Association (AZPRA). It is very important that this form be complete and accurate. If the AZPRA approves your vendor request form, you will be contacted. If you are not approved you will receive your un-cashed check and a letter explaining the decision via the mail. Prior to the event you will receive a vendor packet. You are responsible for the information in the confirmation packet and must follow all the rules and regulations in order to participate in the event. The confirmation packet will have details pertaining to set-up times and locations. The confirmation letter will reiterate that you will be held responsible for the clean up of your booth space and could be fined up to $100 if trash is left in your area or the space/turf is damaged. Please note the fees and deposit due dates. If your fees are not paid in full by the given due date, your space will not be reserved and this will result in additional fees if space is still available. All fees are subject to change at the discretion of the AZPRA. By returning this vendor request form you agree to abide by all the rules and conditions set forth by AZPRA and their officers. The Food Vendor Fee includes the $25 Maricopa County Health Department Fee. All Vendor fees include trash removal, transportation of materials and staff assistance in unloading/loading. AZPRA reserves the right to waive booth fees on a case by case basis. ELECTRICITY: (Available to food vendors only) If you require electricity, there is a $50 fee for 2 outlets. This fee must be included with your payment and agreement. Special electrical needs may be arranged for an additional fee. AZPRA is not responsible for damage to equipment caused by a power surge. INSURANCE: 1. Provide a Certificate of Insurance naming AZPRA as additionally insured, [INSERT APRA ADDRESS] 2. Note: If you have a general insurance policy for your company please provide us a copy. 3. Due 2 weeks prior to the event. If not received by due date your event space will be made available to other vendors. 4. Mail insurance to: INSERT APRA ADDRESS] HEALTH REGULATIONS: All food, beverage and snack vendors are responsible to be aware of and in compliance with all Maricopa County health regulations. Inspectors will be onsite. If you have any questions, please contact the Maricopa County Health Department at XXX-XXXXXXX BUILDING AND FIRE DEPARTMENT REGULATIONS: A special event tip sheet will be mailed to you to ensure compliance with the City of Tempe Fire Department and Building Department regulations. Inspections will be completed by the City prior to the start of each event. LICENSING REQUIREMENTS: All potential vendors for any AZPRA Special Events must have a valid business license on file. Once you receive approval notification on your booth space, if you do not have a business license on file, you will have one week to submit for a license. If you are unsure on the status of your license, please contact us today so we may facilitate this process. We will deny vendor space to any business that does not have a current and valid business license with respect to the city the event is being held in.

Event patrons will be arriving at the same time and most will stay for the duration of the event.
