HANDOUTS FOR ADMISSION AND DISCHARGE

NURSING
Nursing encompasses autonomous and collaborative care of individuals of all age’s families, group and
communities, sick or well and all settings.
HOSPITAL

 A place where people who are ill and looked after by nurse and doctors.
 Hospital is an institution providing medical and surgical treatment and nursing care for sick and
injured people

Admission
- is defined as allowing patient to stay in hospital for observation, investigation, treatment and care
Purpose of admission
- To undergo evaluation and treatment
- To provide emotional security to the newly admitted patient and his family

Types of admission
Emergency admission – Requiring immediate treatment. (Ex. RTA, Poisoning, burn and cardiac or
respiratory emergency)
Routine admission - Patients are admitted for investigation, diagnostic and medical or surgical treatment.
(Ex. Patient with hypertension, diabetes, and mellitus)

- Admission book
- Preparation of paper
- Drug book
- Diet book
- HMIS entry

MEDICO-LEGAL CASES
- are an integral part of medical practice that is frequently encountered by medical officer
- proper handling and accurate documentation of these cases is of prime importance to avoid legal
complications and to ensue that the next of kin received entitle benefits

 ELECTRICAL INJURIES
 POISINING, ALCOHOL INTOXICATION
 BURNS AND SCALDS
 SEXUAL OFFENCES
 ATTEMPTED SUICIDE

DISCHARGE
- Relieving patient from the hospital setting, who admitted as an inpatient in the hospital setting, who
admitted as in inpatient in the hospital
Types of discharge
 Planned discharge.
 DAMA/LAMA (Discharge against medical advice / Leave against medical advice)
 Transfer
 Abscond
 Referral

Discharge Planning
 Nurses play an important role in discharge planning in the hospital.
 Continuity of care is important.
 To achieve continuity of care, nurses use critical thinking skills and apply is centralized,
coordinated interdisciplinary process.

5.Referrals
6. Arranging
Nurses’ responsibility in MLC Discharge
1. Check for medico legal cases
2. Notify medical officer in charge
3. Abscond cases immediately contact medical officer in charge
4. Maintain all documents in proper manner
5. Take in written handing over and takin articles
6. Never discharge patient without order by the physician
