AFFIDAVIT OF UNDERTAKING
I, ROGELIO G. LACYOD, of legal age, single, Filipino and a resident of
OD-382 Banig, Tawang, La Trinidad, Benguet, Philippines, after being duly
sworn to in accordance with law, hereby depose and say that:

1. I am a driver of a Taxi and on September 21, 2019 at around 7:30PM

I was involved in a vehicular accident at Leonard Wood, Baguio City;

2. The taxi I am driving was involved in a vehicular collision with

another motor vehicle. I and the driver of the other motor vehicle were
given a chance to amicably settle, however, we were not able to settle;

3. I tried to contact the driver of the other motor vehicle to try the settle
the dispute; however, despite my many text and calls, he does not
answer or reply. So, I am requesting for the release of my said
DRIVER’S LICENSE so that I could continue drive the Taxi to earn a
living, considering the fact that I stop driving as I have no license;

4. I hereby undertake that I will PRESENT my driver’s license before

the police and other authorities if required or needed;

5. I am executing this Affidavit to attest to the veracity and truthfulness

of all the foregoing and for all legal intents and purposes it may serve.
