Brief Description

On Wednesday 12nd February 2014, 3:00 PM, the RDT FLID MCS - JAR tool couldn't
be pulled after performing pressure testing and sampling the formation fluid in 5331 m
MD. It was stuck on the cable as indicated by down-hole tension reading in RWCH.
The tension was put to maximum allowable 14,750 lbs but still couldn't pull the cable
free. On Wednesday 12nd February 2014, 10:30 PM, it was decided by client to
perform cut-n-thread (strip over) fishing to pull the cable free. Long elevator link
installation in the top drive was requested by engineer on location, but company man
and rig superintendent didn't agree to use long elevator link because of the difficulties
that will arise when connecting drill pipe with the top drive. On Thursday 13th
February 2014, 15:30 WIT, wireline cable was parted during fishing operation. Cable
was under tension 11345 lbs, and parted at top of the pipe stand when picking up pipe
around 1 meter so the roustabouts could remove slips (stand pipe no 28, according to
rig tally). One end of the cable fell inside the drill pipe, and the other end went thru top
sheave wheel and fell down to the rig floor with complete top spear assembly. There
were no personnel injured or in the red zone when this occurred as per JSA. The
safety barricade was also around the rotary table area. After assessing event and
reporting to upper management, proceeded to POOH drill pipe and recovered 100%
of wireline that was sitting on top of no-go connection. Fishing operations was revised
to mitigate cable friction against top drive before continuing to RIH.
