    MEDICAL REPORT

    The above named patient of our hospital is known to be pregnant and in her third
    trimester stage. This was diagnosed due to the patient complains of symptom of immense
    fatigue, muscular aches and acute pains in the body system.

    Due to this her status, situation of extreme physical exertion, immense heat or cold as
    well as environment of rigorous activities should be avoided. Also medical monitoring by
    her physician is essential for the management of her health.

    Hence it is therefore under medical advice that any situation that would place undue
    under strain on the patient should be avoided and she is advised to be on bed rest as her
    delivery date draws near.

    So kindly use your good office in granting her request so as to easily access her family
    Doctor for constant management of her health.
