I, NIÑO MICAEL C. GOLEZ, of legal age, single, Filipino, and a resident of Sto
Niño Ville, Brgy. Mambulac, Silay City, Negros Occidental, Philippines, after having been
duly sworn to in accordance with law hereby depose and say:

1. That I am a bonafide resident of Sto Niño Ville, Brgy. Mambulac, Silay City,
Negros Occidental, Philippines, with good moral standing.

2. That I engage in selling of Motor fuel and oils at Rizal St., Brgy. 5, Silay City,
Negros Occidental, Philippines.

3. That I am not paying any rental fee or any lease fee for the operation of my
said business in the above-mentioned address.

4. That I am executing this affidavit to attest to the truth of the foregoing facts to
the best of my own knowledge and based on authentic records at hand and for the purpose of
declaring that I am not paying any rental fee.
