What were your involvements in the conduct of the buy-bust
operation on ______, if any?
BC:
I was informed by the arresting officer PDEA Agent James Flores on
_____ to attend the pre-operational briefing of the buy-bust operation to be
conducted against Juan G. Dela Cruz which they conducted at the Brgy.
Hall.

PROS:
And what happened at the pre-operational briefing of the buy-bust
operation?

BC:
The PDEA agent informed me that they were tipped by their
confidential informant of a possible buy-bust operation on Juan G. Dela
Cruz and that I should be ready in case they would call me that night to
witness the incident.

PROS:
Did the buy-bust operation happen?

BC:
Yes on ______.

PROS:
And what happened after the buy-bust operation?

BC:
We went to the place where Juan G. Dela Cruz was arrested and then
we went to the police station. I personally witnessed the marking of the
items used in the buy-bust op and saw the items listed in the inventory at
the place of arrest in _____ allegedly seized from the accused and
inventoried by the arresting officers.

PROS:
What happened after the items were listed and inventoried?

BC:
The PDEA agents made a written Inventory Report and had us sign
the Inventory of the Seized Properties and Items form.
