hereby solemnly affirm and declare as under:-

1. That the deponent is the permanent resident of the above-mentioned

address.
2. That the name of the father & mother of the deponent is HABIBAR
RAHMAN MONDAL & AYESHA BEGAM.
3. The name of my husband is MD ABDULLAH MONDAL.
4. Warrant No. CGR320/11, Under Section 341/324/114 IPC, Warrantor –
Waitgung P.S 2020 it is not on my name.
