who is enrolled at Agusan del Sur National High School as Grade/Section
__________________________, SY 2022– 2023 hereby allow our son/daughter to join the
19th National Science Quest through online on March 17-19, 2023 and conduct face-face
review on March 3-4, 2023 at agusan del Sur National High School.
We hereby agree to hold free and harmless all tlhe directors, officers, staff, teachers
and agents of Agusan del Sur National High School, the Division of Agusan del Sur, and the
Association of Science Education in the Philippines from any and all liabilities, damages
and/or claims related/incidental to our son’s/daughter’s joining the competition for the
whole duration.
We hereby certify to the best of our knowledge that all the information contained in
this form is true and correct.
