    SUBJECT: REQUEST OF SICK LEAVE FOR 20 DAY’S (DUE TO


    COVID SUSPECTION)

    Most respectfully it is request that I am suffering from fever, cough and Eye’s

    infection from last week during field work. The doctor suggest me to take

    quarantine it can be symptoms of COVID. So I cannot continue my field work

    in this situation. If I get health before my requested leave I will join back my

    job.

    Kindly grant me leave for 20 days from 26th April to 15th May 2021.
