2. That I have applied for taking the Laguna University Admission Test (LUAT);
3. That up to now, I haven’t received my Form 138 (Grade 12 Report Card);

4. That I will enrol only to Laguna University and will not apply for admission to
any other College or university; that in case that I have a pending application
for admission to any other higher educational institution. I undertake to
withdraw the same upon signing of this undertaking;

5. I will submit the hard copy of the requirements* on or before June 30, 2020 to:

Laguna Sports Complex, Bubukal, Santa Cruz, Laguna

6. I understand that I will be bounded to enrol at Laguna University once I gave

my consent to the Registrar’s Office to register me.

7. I understand that to be admitted to the Board Programs (BEED, BSED, BSA, BSME),

the following conditions are required:

• BSME, BSA, BEED, and BSED - a General Weighted Average (GWA)
of at least 85; no grade below 80 in the Report Card;

• BEED and BSED - preferably a graduate of any strand

• BSA - preferably a graduate of ABM strand

• BSME - preferably a graduate of STEM strand

Incoming First Year Student

• Original Grade 12 Report Card (Form 138)

• Original Certificate of Good Moral Character from the

School Principal or Guidance Counselor

• Photocopy of PSA authenticated Birth certificate

• For female married applicant, submit photocopy of PSA
o authenticated Marriage Contract
• Original Form 137-A (to be requested by LU to the SHS concerned)
• 2 pcs. Passport Size Picture with Name Tag
• Long brown envelope
