AFFIDAVIT OF UNDERTAKING
I, ___________________________, of legal age, ________ and a resident of Barangay
Pasong Putik Proper, Novaliches, Quezon City, after having been duly sworn to an oath in
accordance with law, do hereby depose and state;

1. That I am voluntarily submitting myself for confinement at Balay Silangan Reformatory

Facility pursuant to Dangerous Drug Board (DDB) Regulation No. 2, Series of 2018 for a
minimum of three (3) months and/or depending on the progress of the Boot Camp
course;
2. That I do understand that Balay Silangan Reformatory Facility will not be liable for
anything that would happen to me until basic requirements for admission and
completion are provided and completed;
3. That I firmly follow the rules and policies strictly imposed by the reformation team
inside the Balay Silangan and whatever I commit mistakes they have the right to
reprimand me;
4. That I am doing this act not myself, my family and my community but for the
betterment of my country, the Philippines;
5. That I am executing this Affidavit freely and voluntarily to attest to the truth of the
foregoing facts without any violence, threat or intimidation against me.
