appropriate work visa and/or permit, including an Alien Employment Permit (“AEP”), or
a Certificate of Exclusion, and Pre-arranged Employment or 9(g) Visa from the
Department of Labor and Employment (“DOLE”) and the Bureau of Immigration
respectively, upon [Foreign National Name] arrival in the Philippines;
 6. [Name of company] agrees to be visited and audited by inspectors from the DOLE
and/or Bureau of Immigration in relation to the foregoing, if necessary;

7. This Affidavit of Undertaking is executed in support of the TBE application of [Foreign

8. I hereby declare and affirm that all facts stated herein are true and correct to the best of
my knowledge. Misrepresentation or failure to disclose material facts shall serve as a
basis for the disapproval of this application and may subject me to possible legal action
under existing Philippine laws and regulations.
