I, JEFFREY TABERARA, of legal age, single, and with address at Cabul-an

Capoocan Leyte under oath, states, that:

1. On December 5, 2022, while I was applying for Globe Internet Connection, I

was informed that there was an active account no. 886581873 under my
name with an outstanding balance of Twelve Thousand Eight Hundred
Twenty-Seven and Thirty-Eight Cents Pesos (P12,827.38) with an address at
Park Madasigon, Bobon Mati City, Davao Oriental;
2. I am NOT the same person who opened an account with Globe;
3. I have never been a resident of Park Madasigon, Bobon Mati City, Davao
Oriental, as I am a bonafide resident of Cabul-an Capoocan Leyte;
4. I never opened an account with Globe and this is my first time applying for
the Globe Internet Connection;
5. I attest the truthfulness of the foregoing statements in order to clear my name
and approve my application with Globe.
