This is to reiterate Division Memorandum No. 655, s. 2021 entitled “Submission of Documentary Requirements for Private Educational Institutions Intending to Decrease or Not Increase Tuition and/or Other Fees for SY 2021 -2022 and the School Years Thereafter,” content of which is self-explanatory. All concerned private educational institutions are informed of the following documentary requirements to be submitted to the Regional Office, in one (1) folder: a. Schoo!’s letter of informing of its intent to decrease or not to increase its tuition and/or other fees; b. Statement of itemized current rates of tuition and other charges and of proposed allocations of the incremental proceeds. Such statements shall, when accomplished be under oath by the proper official (s) of the concerned school; . Preceding year’s confirmation of tuition and/or other fees issued by the Regional Office; |. Financial statement showing the financial status of the school duly certified by a certified public accountant; and .
