2022informed Consent Form For International Students of HUST
【Conditions for applying to return/come to HUST】 Students should receive the COVID-19 vaccine (including
the booster vaccination)and possess a certificate of “negative” for PCR test of COVID-19 and the serum-specific
IgM antibody test. Students shall purchase the Comprehensive Insurance for International Students in China before
departure. Tuition and accommodation fee are required to be paid before registration, which is the precondition of
getting the residence permit.
Due to the long quarantine time, students applying for returning to HUST need to make sure that they are fully
prepared for this travel physically, financially and mentally. Students who are prone to anxiety and psychological
distress must consider seriously before departure.
【Expenses】 All expenses incurred by returning/coming to HUST, including the insurance fees, transportation fees
(such as fees incurred by international flight, inter-city transportation and possible close-looped transportation),
accommodation fee, meal fees, PCR test fees, and the possible expense incurred by COVID-19 diagnosis and
treatment shall be borne by the students themselves. If the scholarship covers parts of those fees, please confirm with
scholarship administration organization. Students returning/coming to HUST must prepare sufficient funds and be
clear about the scholarship policy, and HUST will not provide any financial aid.
【Family Member and Accommodation】 As per the policy of MOFCOM students (Scholarship No. with MPA or
MOC), they shall not bring family members and must live on campus. For students returning to HUST, it is not
authorized to issue VISA for family members. For the students with family members currently in China, they shall
live off campus.
【Back to HUST Process】 Students should send the signed copy of this form to the corresponding coordinator by
Email, and then will get JW201/202 form, Admission Notice (New students and students first entering China), or no-
objection letter (Current Students). Students should keep in contact with the coordinator and follow the instructions
strictly. Quarantine policy: If the arrival city of China is Wuhan, students need to conduct a 7-day hotel quarantine
and a 7-day health monitoring; if the arrival city of China is not Wuhan, students need to conduct a 7-day hotel
quarantine and a 10-day health monitoring. The policy is subject to change and will be implemented accordingly.
I have carefully read and understood the above content, and confirmed that:
1. I am fully vaccinated and have the certificate of “negative” for the PCR test of COVID-19 and the serum-specific
IgM antibody test;
2. I will purchase the health insurance for international students before entering China; The CSC students can
reimburse the payment after arriving in HUST;
3. I will unconditionally abide by China's epidemic prevention and control measures and the regulations of returning
to HUST;
4. I will undertake all the costs related to the returning process and quarantine;
5. I will pay the full tuition fee for one academic year before applying for the residence permit (self-financed students
and scholarship students whose funding period has ended);
6. I promise not to bring any family members to China, not to rent an apartment off campus without ISO’s permission,
and not to leave the campus unless necessary.
