LETTER OF CONSENT

Respected Sir/Ma’am,

We are PS4 researchers from 10 Darwin in Quezon City Science High School currently conducting our
research entitled FitCare: A Non-Equipment-Based Exercise Application with AI Voice Command (FIN) for
Generation X. In line with this we are asking your permission to be one of our respondents in using our FitCare
application. The purpose of this study is to attain knowledge on the satisfaction and navigation of the app for
the user. Your participation in this study is entirely voluntary and you may withdraw at any time.

Your name, age, and birthday will be obtained in connection with this study but with confidentiality. We
are writing this letter to seek your kind permission for you to use the application and document while using it.
Before participating make sure to have an android phone and enough storage to download the app. After that,
we will be surveying your experience in FitCare. The above-said survey could help us gain better knowledge
about the user’s experience/user interest in using the application FitCare.

Darwin in Quezon City Science High School currently conducting our research entitled FitCare: A Non-
