The Centre Educational Support Programme

Dear Parents,

Thank you very much for participating in The Centre Educational Support Programme.

The Centre is a social enterprise that supports companies to deliver improvements within their
supply chains that benefit workers, families and children. We would hereby like to introduce you
to this programme and help you understand the background, purpose and support that it entails.
We have both agreed that it is currently not the right time for your child to be involved in work
in [country], and the best option is for your child to continue with education, which will be
beneficial to his/her future in the long-run. Thus, we have developed this educational support
programme (the Programme) with our client’s financial support.
The details of the Programme include:
1. The Programme will start from _______(Year) _______(Month) _______(Day) to
_______ (Year) _______ (Month) _______(Day) (the “Programme Period”).
2. The financial support under this special Programme includes living stipends
USD______ per month (USD______ in total), and tuition fees up to USD______,
which will be paid according to actual education expenses.

The Centre will support the school searching and enrolment of your child____________ (Name
of the child), transfer the living stipends and tuition fees regularly every month and provide
suggestions and consultation if there are any other needs related to the child during this period.

In the event that your child enters employment during the programme period, The Centre will
stop paying for the aforementioned education and living stipends under the programme.

We hope to obtain your consent on the following matters. Please read the following terms
carefully.

A Consent Letter

Throughout the programme, The Centre requires your support to ensure successful
implementation of the programme.

As the legal guardian, you shall be responsible for

 a. supervising the child to ensure he/she does not enter employment when he/she
attends school or training;
b. cooperating with a school to ensure the child complies with relevant codes of
conduct, laws and regulations, and the school/training institution’s security
management rules.

Please also note that we will follow up on the latest progress and report to the client, to ensure
that the progress is in line with the programme plan. However, we will not disclose any
confidential information to any third parties, including our client who provides the educational
funds. This means we will not disclose full name, ID number and images of the child and family
members, family address and contact information of child and family members, school name,
address, teacher’s contact information and bank account.
We hereby would like to seek your consent in allowing The Centre to share the necessary data,
including the latest updates and progress of your child in regards to the educational support
programme.
By signing this form, you as a legal guardian of child have agreed to the above terms, and you
also have asked for your child’s consent to sign on their behalf. If you have any questions or
