How to Access Office 365 Forms and


Create an Online Questionnaire
Providing that they are enrolled to the University, all Students and Staff have access to the
Office 365 Subscription.

1. Type the following link on the address bar of your browser: www.office.com

2. The following page should be presented to you;

3. Press the Sign in Button located on the page;

4. The following page should be presented to you;

5. You may Sign in with your Staff ID. You may not be asked to enter your Staff Password.
Note: Please include the @gre.ac.uk at the end of your Staff ID;

6. After Signing in successfully, you will be presented with the following page;

7. From the blue menu, click on the Forms Icon;

8. You will now be presented with the following page;

9. Click on the New Form Button;

11. Click on the Form Title to change the title of the questionnaire – you may also want to give it
a description;

12. Once you have provided a quiz title and a description (optional), you can press the Add
question button;

13. Once you have pressed the Add question button, you will be presented with four options.
The various options display how you would like the user to respond to the question. This
would be in a form of a Choice Box, Entering Text, providing a Rating and Choosing a Date
from the Calendar;

14. You may now choose the response type, for this example, the choice box will be pressed;

15. You may now Enter the Question and its options. If you would like to add additional options
you may do so by clicking on the Add option. You may also allow multiple answers and make
it a required field – they would have to enter a response before clicking submit. Below is an
example of a choice question;

16. You may also add additional questions by clicking on the Add question button;

17. Below is an example of a completed questionnaire;

18. Once you have completed your questionnaire, you can Preview it by clicking on the Preview
Button at the top of the page or click Share.

19. You may now share by copying and pasting this link to your responders or email your
recipients by clicking on the email icon;

How to Access Form Responses and Download Responses to a Spreadsheet

1. To Access Form Responses, go to the Quick Access Menu and click on the form you would
like to view the responses for:

2. Once you have chosen your desired form, you will be presented with the following page:

3. From here, click on the Responses Tab:

4. Once you have clicked Responses Tab, you will be presented with the following page:

5. Here you would be view the statistics of your responses and Open in an Excel Spreadsheet.
Please Note, to download the responses via a spreadsheet you must have at least one
response.

6. To download the responses via an excel spreadsheet, simply click on Open in Excel:

7. Once you have pressed on this button, you will notice a file being downloaded. Locate your
downloads folder and open the Excel Spreadsheet that you just downloaded. The name of
the File would be the form title. All responses and data will be stored onto an Excel
spreadsheet.
