100% Membership Excellence Award Application

The 100% Membership Excellence Award is to recognize those advisers who recruit all of their students during
an entire school year to join FBLA. This includes all classes in both fall semester and spring semester.

Advisers who achieve this honor with a minimum of 75 students total will receive a special gift and a plaque at
the State Leadership Conference

You must attach to this application:

 A statement on official school letterhead from a school administrator or registrar listing each course
(including non-BCS courses) taught during the entire school year. If courses are yearlong courses, this must
be clearly stated.
 Class rosters for each class for each semester (if a yearlong course, you must attach a fall semester class roster
and a spring semester class roster)
 Your national FBLA membership roster with all students’ names who appear on the class roster highlighted.
All dues must be paid and received by application date.
