PUERTO/JCT SAYRE HIGHWAY), PACKAGE 7, CAGAYAN DE ORO CITY, CAGAYAN
DE ORO CITY.

Please be advised that this Office does not issue a certificate of approval for a
[completed/almost completed] construction project.

Upon evaluation of the submitted documents, however, we noted that while the project is
[completed/almost completed], you have complied with the minimum requirements as
reflected in DOLE Department Order No. 13-98 or the Guidelines Governing Occupational
Safety and Health in the Construction Industry and DOLE Department Order No. 198-18 or
the Implementing Rules and Regulations of Republic Act No. 11058 or the OSH Law.

You may submit this letter to _______________________________________________

for whatever legal purpose this may serve, including billing and collection.
