AFFIDAVIT OF UNDERTAKING BUSINESS PERMIT

I, A, Filipino, of legal age, married and a resident of _, Philippines, after

being duly sworn in accordance with law, depose and state that:

1. I am an applicant for a Business Permit before the City Government _ for

our family business which is a boarding house at our address stated
above;

2. I undertake to observe and do the following, in support of my business

permit aplication before the City Government of _:

A. Not to accept nor enter into any lease contract agreement involving
lessee/s who has/have motor vehicle/s;

B. To construct a parking garage for one vehicle at the boarding house

premises starting this _;

3. I execute this affidavit in support of my Business Permit application before

the Business and Licensing Division in _and for legal intents and
purposes it may serve.
