Sora International Preschool of San Carlos

2023-2024 Admission Agreement


-Full Day/School Day Program-
The following agreement is made by and between (hereinafter referred to as
the Parent) and Sora International Preschool (hereinafter referred to as the School) for your child
(hereinafter referred to as the Child).

Initials Agreements
Acceptance of Admission Agreements
I have read, initialed, and signed this document.

Parents Handbook:
I have received a copy of the Sora International Preschool Parents Handbook. I have
read and understand its policies, protocols, and procedures, and agree to comply with
the program rules and regulations.

Provided Services:
I understand that the school provide the 5 days (Monday-Friday from 8:30am to
3:30pm for School Day program and 5:30pm for Full Day program) of the preschool
program.
• School Day Program (8:30am-3:30pm): $2,040
• Full Day Program (8:30am-5:30pm): $2,350

The school provides leaning activities in an age-appropriate way, supervision and


care during the school hours. The school provides snacks in the morning and
afternoon. The Parent provide the Child’s lunch.

Release of Children:
I understand that the Child will be released only to myself, a legal guardian, the other
parent (except were prohibited by court order), and to those persons whose names I
have listed on my child’s Emergency Form (LIC700 Identification and Emergency
Information). I understand that if someone other than the Child’s parent/guardian are
to pick-up the Child on a specific day, I must notify the school, even if they are listed
on the Emergency Form. The designated person will be required to show a picture ID
upon pick up.

Illness:
I understand that I will be contacted should the Child become ill while at school. I
agree to pick up the Child promptly within 30 minutes upon such notification. I also
understand that the Child cannot attend the school on a day in which they are ill, as
defined in the Parents Handbook.

Communicable Disease:
I understand that I must inform the school within 24 hours, or the next business day
after the Child or any member of the family has developed any communicable disease
including but not limited to possible COVID-19 exposure. I also understand that life-
threatening diseases including but not limited to COVID-19 must be reported to the
school immediately.

Medications at School:
I understand that if the Child requires medication during school hours, it must be kept
in the school office in its original packaging labeled with the Child’s name, and
necessary documentation must be completed by the parent/guardian and prescribing
physician.

Emergency Contacts:
I understand that I am required to maintain at all times, at least one additional
emergency contact other than myself, including full names and phone numbers. I
understand that in the event of an emergency for which my emergency contact or I
cannot be reached, the staff may contact the police or other emergency authorities.

In the Event of an Emergency:


In the event of an emergency, if I cannot be reached, the school has my permission to
contact the Child’s physician(s) listed on the Child’s Emergency Form, or have the
Child transported to a local hospital for care. I will not hold the school or its
employees liable. I understand that I am responsible for all expenses incurred.

Change of Information:
I understand that it is my responsibility to inform the school of any changes to the
information on the Emergency Form, as well as other forms in the registration packet,
including, but not limited to: address, home phone number, the parent/guardian’s
work numbers, medical conditions, and pick-up authorizations.

Extended Care:
I understand that Extended Care is available at $20/hr when space is available.
- Extended care needs to be requested between a month and a week before.
- Notify school of your program change 30 days in advance.
- The Fee is not refund if child gets absent, change the schedule.

- The fee will be calculated each child's Extended Care hours of the month (Due: on the
25th of each month) and included in the monthly invoice.
- The period between the 26th and 25th of next month will be calculated each month.
For example, the period of 10/26-11/25 will be charged on December's invoice. 11/25
is the due date.
- In an emergency, school accept short notice if asked for it a day before, but if school
has a spot available.

Tuition and Refund:


I understand the following tuition policies.
- A $400/student Annual Fee is due with the August monthly tuition payment. The
Annual Fee in non-refundable.
- The Annual Fee includes all the necessary miscellaneous cost for the 2023-2024
School Year (8/14/2023-6/7/2024).
- A $100/ New students Enrollment fee is due with the first enrollment costs. The
Enrollment fee is non-refundable. The Enrollment fee includes the Art box, sheets,
and tote bag.
- The brightwheel app account is used to pay the tuition and other necessary fees.
- Tuition Payment is due on the first day of each month.
- A $25 late fee will be imposed on all tuition received after 3pm of the due date. If
tuition is not paid by 3pm on the 5th of the month, the Child will not be allowed to
attend preschool.
- There is no reduction of fees for absences and vacations.
- In case of long-term school closure due to emergency circumstances, the tuition will
be reduced by 30% from the previous month. The tuition already paid for the ongoing
month is non-refundable.
- understand the following tuition policies. services and fees will be charged and added
to the invoice of the following month’s tuition payment.

Tuition Deposit:
I understand that I must submit a non-refundable, non-transferable Tuition Deposit
upon enrollment for the program. The Tuition Deposit will cover the payment for the
last 4 weeks of the School Year. In the case of a withdrawal before the completion of
the School Year, the Tuition Deposit will not be refunded.

Changing Program:
I understand the following policies.
- To change program from Preschool Full Day Program to School Day Program, I must
notify the school 30 days in advance.
- School accepts your change only if the space is available.
- When changing from Preschool Full Day Program to School Day Program, the
deposit was NOT refunded.
- When changing from Preschool School Day Program to Full Day Program, you will
be charged the regular fee minus the Preschool Day Program deposit.

Late Pick-Up Charge:
I understand that there will be a late pick-up fee of $1 per minute after 3:40 pm for School
Day Program and 5:30 pm for Full Day Program/Extended Care. The fee will be charged
on the next monthly invoice.

Withdrawal:
I understand that I may withdraw a Child from the school and terminate the
agreement after submitting a written request made 4 weeks prior to the date such
withdrawal. I understand that a parent’s termination request will not go into effect
until 4 weeks after the date of submission of the written withdrawal request and
therefore will be charge for the 4 subsequent weeks after the date of submission.
I understand that if my child withdraws before the end of the school year in June, the
Tuition Deposit will not be refunded.

Violent or Unsafe Behavior:


I understand that I may be contacted should the Child display violent, unsafe, or
continually inappropriate behavior. I agree to pick up the Child promptly upon such
notification.

Uniform and Clothing Policy:


I understand that the Child must wear the school uniform on Monday, Tuesday, and
Wednesday. I will follow the instructions from the Parents Handbook regarding the
appropriate dress code.

Conditions of Termination
The School has the right to terminate this agreement and ask the Parent to withdraw
the Child’s enrollment if any of the following occurs:
-The Child presents a health or safety threat
-Repeated discipline problems
-Lack of parental cooperation
Parents involvement in inappropriate and negative actions that could affect
the reputation of the school.
-On-going late pick up
-Inappropriate parental behavior to children, teachers, and/or staff
-Lack of tuition payment

The School will give the Parent a 3-step advisory as follow before the termination of
the agreement occur;
1) a verbal warning
2) a written warning
3) an in-person conference

In case of termination due to the reasons above, the tuition deposit and the tuition the
Parent already has paid for the month will not be refunded regardless of the date of
termination.

Rights of the Licensing Agency


I understand that the California Department of Social Services, Community Care
Licensing Division has the authority to interview children or staff without prior
consent. They also have the authority to inspect, audit, and copy child or childcare
center records upon demand during normal business hours.

Photo Release
Participation in preschool sponsored activities constitutes permission to use photos of
participants for promotional purposes in all print, electronic and password protected
web media and related organization promotions.

Terms of this Admission Agreement


I understand that this Admission Agreement is valid for the Summer Program 2023
and the 2023-2024 school year, or when the Parent withdraws the Child with a 4-week
notice. The school will notify the Parent at least 30 days in advance in case of any
changes or modifications of this contract including but not limited to tuition rate,
school operation days/hours, and health policies.
