I/We hereby fully understand and acknowledged that by signing below, I/we hereby agree to order the accessory(s) mentioned above
and shall comply with the following terms and conditions:-
1) Upon any purchase of the Coway’s product CHP-7310R Ombak, the customer may submit this form together with the Sales Order
Form to the Coway representative to order the accessory(s) mentioned above.
2) The mineral filter & faucet shall be installed by Coway Technician during the product installation provided that this order form
is submitted prior to product installation.
3) Any order of accessory(s) mentioned above after the product installation must be communicated to the Coway Lady / Service
Technician during Heart Service. This form shall be submitted to the Coway Lady / Service Technician upon performance of Heart
Service.
4) The filter-changing period and the warranty period of the mineral filter is every six (6) months subjected to the date of installation
of the mineral filter. There will be a sum charge upon the filter-changing.
5) The warranty period of faucet is twelve (12) months subjected to the date of installation of the faucet.
6) If the customer does not intend to renew and replace the mineral filter after the expiration of the accessory(s), Coway has the
right to retrieve the accessory(s) during the performance of Heart Service.
7) The payment of the accessory(s) shall only be accepted in cash and collected by the Coway Technician, Coway Lady or Service
Technician. A receipt must be given to the customer upon the collection of the payment.
8) The order of the accessory(s) mentioned above shall only applies to Coway’s product CHP-7310R Ombak.
9) Coway at its sole discretion has the right to revise and amend the terms and conditions from time to time without prior
notice.
Personal Data Protection Act 2010:
(a) I/We The customer hereby fully understand and give consent to COWAY (MALAYSIA) SDN. BHD. to collect, store and
process any personal information for the purposes of receiving updates, news, promotional and marketing mails or
materials from Coway in accordance to Coway’s Privacy Notice which details can be found on Coway’s website at
www.coway.com.my/privacy-notice (“Privacy Notice”)
(b) Coway may update its Privacy Notice from time to time and may use reasonable efforts to bring any significant changes to
the Privacy Notice to the customer’s attention.
10) The terms and conditions in this form shall be deemed to be incorporated into the Sales Order Form and shall form an integral
part of the Sales Order Form
11) In the event of any inconsistency or discrepancy between any of the provisions of the Sales Order Form and the Terms and
Conditions in the Accessory Order Form, the Terms and Conditions in the Sales Order Form shall prevail.
12) For avoidance of doubt, all other terms and conditions stipulated in the Sales Order Form shall remain unchanged, save to the
extent varied by the terms and conditions of this form.
