    Dear Ms. Macapil:

    I am writing to inform your good office that I have been absent from work from
    Dec 1-3 2022.

    The reason behind my absence from the office for the last two days was a throat
    infection or follicular tonsillitis.

    The said infection caused me to have a high fever and loss of appetite. I was
    accordingly advised by the doctor to rest at home for two days. I sent an text
    message and informed my department head about my concern who allowed me
    to rest while recuperating at home.

    Please find attached doctor's prescription along with a copy of my


    correspondence with our head regarding my leave from work.

    Kindly accept my letter to document this leave.
