I, SEVELINA ADAM KABILONDO an adult, Christian and resident of, BURONGE
KIGOMA do hereby solemnly Swear and sincerely state as follows:
1. That I am a citizen of the United Republic of Tanzania.
2. That, Iam the owner of Telephone No.0767193725 with a name of SEVELINA ADAM
KABILONDO registered by using My National Identity Card (NIDA) with
No.19890520-47116-00003-13
3. That, previously I registered a Vodacom telephone No. 0767193725 by Using another
Nationa Identity card (NIDA) with No. 20000307-47313-0000-12, beared the names of
GOODLUCK AMOSI RAJABU.
4. That I curently change the ownership of the said number from the original owner to my
possession.
5. That, before changing of ownerhip the number had a transaction amounting of Tanzania
Shilings Fifty Six Thousands. (Tsh 56,000/=)
6. That, by making this affidavit I do authorize and requsted the authority concerned to
deposit back the remaining balance, so baried with the number from the original owner to
my account Vodacom number 0767193725 with the new name SEVELINA ADAM
KABILONDO
7. That I make this affidavit pursuant to Oaths (Judicial Proceedings Declaration Act of
1960), (R.E 2019)
8. That I, SEVELINA ADAM KABILONDO verify that what is stated in paragraph
1,2,3,4,5, 6 and 7 are true to the best of my knowledge.
