The DONEE, for and in behalf of the school whose names are cited in the enclosed list/ certification, hereby
accepts the donation from the HPTA herein represented by SHALIMAR T. BATAN, PRESIDENT thru the
Adopt-a-School Program, donation package composed of the following:

1 pc sterilization box Php 1682.75 , 1 large ,1 medium,1small pail and 1 deeper Php 505.00 and labor for the
maintenance of the ceiling fan and lights of the grade 3 room Php 200.00, labor for the sewing of 2 small curtain
Php 200.00 with the total amount of Six thousand nine hundered sixty-two pesos and seventy-five centavos
only. (Php 6,962.75)

For and in behalf of the recipient schools, the DONEE expresses his most sincere appreciation for the
benevolence shown by the DONOR.
