University with business address at Lucban, Quezon after having been subscribed to an oath,
hereby depose and state:

1. That I hereby certify that Jon Bennet R. Calupig has taken his/her apprenticeship at
Southern Luzon State University Tayabas Campus
and has completed 240 total number of hours;

2. That, the apprenticeship program comprises of the following training; Electrical Inspections
:Audit Report and Propose Electrical Design Revision

3. That Jon Bennet R. Calupig is hereby qualified to undertake the Registered Master
Electricians Licensure Examinations;
