Respectfully suggesting to discontinue Tolvaptan 15 mg ¼ tab OD

Discussion:

The use of sodium chloride in combination with tolvaptan may result in a too rapid correction of
hyponatremia. Tolvaptan may cause the level of sodium in your blood to increase too quickly.

Recommendation:

Respectfully suggesting to monitor suspected bleeding when taking enoxaparin and fish oil, and to
add Protamine Sulfate when bleed occurs.

Discussion:

Enoxaparin is use in caution in patient with uncontrolled HTN and possible cause of decreased in
blood cell counts. Protamine sulfate would help to reverse enoxaparin associated by bleeding or
hemorrhage. If bleeding is suspected, anti facto Xa can be measured to adjust the dose of Enoxaparin
accordingly. Fish oil may enhance the anticoagulant effect of enoxaparin

Recommendation:

Respectfully suggesting to add drug for VAP-MDRO Carbapenem. Culture and sensitivity testing
must done before initiating the therapy

Discussion:

Extended spectrum B lactamases represent a major source of antimicrobial resistance in gram


negative bacteria so carbapenems have greater stability against ESBLs and are good choice to treat
infections due to ESBLs.
