I Mr. Ganesh Dutt Sharma, the authorised signatory for m/s APS International, “The Firm”, a
proprietorship firm having its registered office at E-148, Phase VII, Industrial Area, Mohali have
availed a working capital limit / loan of Rs. 2.60 Crores (Rupees Two Crores Sixty Lakhs) from ICICI
Bank Limited (“ICICI Bank”), Phase VII, Indl Area Mohali Branch
Branch which has been sanctioned to us vide _____________ dated __________________.
I/ We understand that Government of India, Ministry of Micro Small & Medium Enterprise
(MoMSME) has launched the ‘Interest Subvention Scheme for Incremental Credit to Micro, Small
and Medium Enterprises [MSMEs], 2018’ (“the Scheme”) and I/ We are desirous of availing the
benefits of the Scheme. We hereby declare and confirm that the Firm/ Company possesses valid
GSTN Number and valid Udyog Aadhar Number (certificates enclosed), details of which are as
follows:

2. Udyog Aadhar Number (UAN) : PB20B0018625

In this regard, I/ we hereby declare, undertake and confirm that:

1. The Firm / Company is a “small enterprise” as defined under the Micro Small and Medium
Enterprises Development Act, 2006.

2. The Firm / Company has availed working capital loan / limit from the following banks/
financial institutions:

in writing.

In case of any claim/s received by ICICI Bank from Small Industrial Development Bank of India
(“SIDBI”) towards false representations or discrepancy in our documents submitted the purpose of
availing the Scheme, I/ We hereby undertake to indemnify ICICI Bank to full extent without any
delay or demur. In the event of the Firm / Company availing the Scheme and/ or any benefits under
the Scheme wrongly due to non-eligibility or for any other reasons whatsoever, I/ We authorize ICICI
Bank to debit our account numbered 342105500198 with the amount of interest subvention under the
Scheme (earlier credited in the said account) along with any other charges/ fees/ penalty, if applicable,
towards refund of such wrongful interest subvention benefit amount to SIDBI.
