    salary (last pay) from your good office, for the reason that I would be unable to claim it personally
    because I am not currently present in the city.

    To support my claim, I have appended a photocopy of my identity ID card (Philhealth ID) to


    serve as a proof that I have allowed the bearer of this letter to make claims on my behalf.
